The report also included a description of the plaque unveiling, page 10: "a handsome bronze tablet commemorating the gift of Andrew Carnegie, Esq., to the City of Toronto was unveiled by the Honourable Mr. Justice Kelly in the absence of the Honourable Chief Justice Sir Glenholme Falconbridge, who had been announced to perform this ceremony."

Plaque text: "This building was erected/through the generosity/of Andrew Carnegie Esq/The cornerstone was laid/on November 27th 1906 by/Chief Justice Sir Glenholme/Falconbridge and the building/was opened for/service on October 28th/1909 by H. T. Kelly Esq. K.C./The members of the Library Board during/this period were Sir/Glenholme Falconbridge/Messrs H. T. Kelly K.C./W. T. J. Lee, Thomas W. Banton/A. E. Huestis, John Turnbull, A. B. Gash, K. C. Thomas, W. Self/and the Mayor of Toronto.” Note: original text was in block capitals.
